Als je in Utrecht iemand traint of beoordeelt in de mate waarin ze bekwaam zijn om zelfstandig een specifieke handeling (EPA) uit te voeren aan een dier binnen onderzoek of onderwijs, moet je daarvoor benoemd zijn als supervisor en assessor bij de IvD Utrecht.  Via dit formulier kun je jezelf als zodanig aanmelden.
If, in Utrecht, you train or assess someone on the level of independence that they can perform a procedure (EPA) on laboratory animals, you need to be appointed as supervisor and assessor by the Animal Welfare Body Utrecht. Using this form you can apply for this.
 
Het aanleren of het beoordelen van competenties in het doen van handelingen of procedures aan (proef)dieren is een grote verantwoordelijkheid en dient professioneel te gebeuren. Enige basis in didactiek is daarbij vereist naast het goed beheersen van de handeling of procedure zelf. Sinds februari 2026 is het niet meer mogelijk als supervisor of assessor op te treden als je niet een daarvoor juiste minimale training hebt gevolgd. Indien je dit formulier inzendt wordt contact met je opgenomen hierover. Je wordt echter nog niet geregistreerd.

To supervise or to assess how to procedures in laboratory animals are performed is a major responsibility and must be carried out professionally. This requires a basic understanding of teaching methods, in addition to expert execution of the procedure itself. Since February 2026, it is no longer possible to act as a supervisor or assessor unless you have completed the appropriate minimum training. If you submit this form, you will be contacted regarding this matter. However, you will not yet be registered.

Als je het certificaat van Module 27 van ETPLAS heel recent hebt behaald kun je die ook opvoeren voor je leven lang leren tijdens de herrigestratie.
